    Jakarta| Air accident investigators have uncovered a chain of errors that led to the Lion Air crash in Indonesia that killed all 189 crew and passengers, starting with Boeing design and certification problems and ending with poor communication between two panicked pilots.

    The Lion Air flight was on route from Jakarta to Depati Amir Airport in Pangkal Pinang when it came down in the water in Tanjung Karawang, West Java, killing all on board. Not for syndication
